This notice covers only the Atlasano interest signup on this website. Atlasano is in development. This form does not create a healthcare account or a patient relationship.

What we collect and why

The U.S.-resident interest form requires first name, last name, email, state or territory, a self-reported U.S. residency confirmation, and separate permission for launch and development updates. Names help us address and identify interested people; state or territory helps us understand geographic interest. This is an interest list, not identity verification or an offer of healthcare services.

When you submit, your browser sends these fields, the consent and privacy notice versions, submission time, and a security-check response directly to Web3Forms. The browser uses a public form access key, not a private email credential. This site no longer saves submissions in its own D1 database.

Please do not share health information. These forms do not ask for symptoms, diagnoses, records, or medical details. They do not create a patient relationship or healthcare account.

Your choices and requests

Signup is optional. Both required confirmations are unchecked by default. You can use the private request form below to request withdrawal, deletion, access, or correction of signup information. This form sends only your email, selected request type, privacy notice version, submission time, and security-check response through the same provider. It does not automatically fulfill a request or verify identity. An operator must review and, where appropriate, confirm the request before acting. Future update emails need unsubscribe handling before they are sent.

Where information is handled

Cloudflare hosts this website. Web3Forms processes submissions, spam checks, and email delivery; Google Gmail stores operator notification copies. hCaptcha provides the security challenge. These services may process IP addresses, browser/device information, and delivery or security metadata. The application does not add analytics or advertising scripts and does not save form entries in browser storage.

Web3Forms’ privacy policy describes international processing, including India, and infrastructure providers such as AWS, Cloudflare and Hetzner, plus spam processing services including CleanTalk and Akismet. See hCaptcha’s privacy policy for its handling. Data may be processed outside the United States. Authorized Atlasano operators use received information for the purposes described here; we do not sell it.

Retention

Atlasano operators will remove their signup and request email copies within 180 days of receipt, or earlier after an accepted deletion or withdrawal request, subject to necessary request handling. This is an operator procedure, not automatic database deletion. Web3Forms has its own retention: its privacy policy permits storage for up to three years in some circumstances. Atlasano’s 180-day operator period does not override provider retention. Requests involving provider-held copies must also be handled with the relevant provider.

Repeated submissions may produce duplicate emails and do not update earlier copies automatically. Use the correction request type for changes. There is no automatic deduplication, account change, or deletion through these forms.

Submission and safeguards

A successful message means the form service accepted a submission; it does not confirm delivery to the operator’s inbox. A network timeout can happen after acceptance, so retrying may send a duplicate. The website does not retry automatically. hCaptcha must be enforced in the provider dashboard before forms are enabled. No online service guarantees absolute security.
